Returning Student-Athletes

Dear Monarch Student-Athlete,

Welcome to Methodist University Athletics! It's a great year to be a Monarch! Toparticipate in intercollegiate athletics at Methodist we need you to complete some medical paperwork. This paperwork involves a Returner Participant Medical Questionnaire, Sickle Cell Test Results and submitting front and back copies of your insurance card.

ALL paperwork must be turned into the Certified Athletic Trainers BEFORE you will be allowed to participate. Listed below is a check list with the required paperwork.

______ Returning Student Participant Medical Information

This information is to be completed online. Listed below is the website to access forms, along with ID and Password information for first time login. When you login and complete the first portion you will need to create a new ID and password. You will need to fill out all information under each tab General, Medical History, Insurance, Contacts and Athlete Forms (Required forms below). We ask that you be as detailed as possible, if we do not have the correct information we will not be able to clear you for participation.

At the bottom of each form there is a box which you must use your mouse or stylus to sign the form then underneath type your name please be sure to sign and type your name. If you are over 18 and do not need your parents to sign just put N/A in signature and typed box for parents. If you do not sign and type your name the form will not save. If you do not answer all the question the form will not save.

We will need a front and back copy of your current insurance card. The insurance card can be submitted by scanning and attaching to an email, by fax or by mail.

_______ Completion of the ADHD / ADD Medical Exception Form

Your prescribing Physician must complete this form if you are taking any form of ADHD/ADD medication. Once the form is completed it can be submittedby email attachment, by fax or by mail. The forms can be accessed on MU athletic website under athletic training, requested from Athletic Trainer or in our online system under e-file forms. If you are not currently taking ADHD/ADD medication you do not need to have this form completed.

______ Sickle Cell Test Results

The NCAA recommends that all student athletes have knowledge of their sickle cell trait status before the student athlete participates in any intercollegiate athletics event, including strength and conditioning sessions, practices, competitions, etc. It is possible to request test results from your primary care physician or the hospital in which you were born. Athlete can be tested at Operation Sickle Cell in Fayetteville for a small fee. All student athletes that choose not to be tested are required to complete a waiver form. If completing the waiver you will be treated as though you are sickle cell positive in practice. As well as, be required to attend educational training about sickle cell that is offered to all athletes who are sickle cell positive. We strongly encourage obtaining your results or being tested. You most likely completed this step before the end of the spring semester) If you have any questions on this requirement please connect the athletic training offices

We ask that you complete the forms by August 1^st. We will periodically send updates of continued missing paperwork to your Methodist University email. We ask that you check your Methodist University email for further updates on information we may need you to submit.
